We provide state-of-the-art tests and procedures to help make an accurate diagnosis and create a treatment plan. We see patients with very common heart problems, such as angina, artery problems, valvular heart disease, and heart failure. We also treat patients with some very rare forms of heart disease.

  • JAMES V TALANO, MD, MM, FACC

    Dr. Talano 
is the Clinical Director and Founder of the SWICFT Institute. He is board certified in internal medicine and cardiovascular disease and served as Chief of Cardiology and Professor of Medicine at Tulane University Medical School, Director of Cardiology at the Cleveland Clinic Naples, Director of Cardiology Graphics and Chief of Clinical Cardiology at Northwestern Memorial Hospital and Professor of Medicine at Northwestern University Medical School and at Lakeside Department of Veterans Affairs Medical Center in Chicago, IL. Dr. Talano is a Fellow of the American College of Cardiology and the American Heart Association. Dr. Talano received his medical degree from Loyola University Medical School. He conducted his residency and cardiovascular fellowship at Georgetown University Hospital and the National Heart, Lung and Blood Institute in Washington DC and also served a clinical fellowship in Cardiology at Tufts University Hospital in Boston. In addition, he holds a Masters in Management degree from the Northwestern University Kellogg Graduate School of Management and is a retired Major in the US Army Reserve Medical Corp. Dr. Talano has authored over 150 original articles, chapters, and review articles.

  • ROBERT BAILY, MD FACC

    ROBERT BAILY, MD FACC 
has joined us from the Pinnacle Health Cardiovascular Institute in Harrisburg, Pennsylvania.  He is a graduate of Northwestern University Medical School in Chicago, Illinois where he originally met and trained under Dr. Talano. Dr. Baily completed his Residency and Fellowship, Research Fellowship and Chief Fellowship training at Penn State University Hospital and his teaching appointments include the Milton S. Hershey Medical Center and Penn State University Hospital. After completing his training he was appointed Assistant Professor of Medicine in the Division of Cardiology and has published several peer reviewed articles in the field of integrative physiology during his tenure at the Pennsylvania State University Hospital. He later served as the Chief of Cardiology, Holy Spirit Hospital in Camp Hill, Pennsylvania for more than a decade and was also elected Chairman of the Medical Executive Board for Holy Spirit Hospital. Dr. Baily is Board Certified in Internal Medicine and Cardiovascular Disease and is a Fellow of the American College of Cardiology, the American Heart Association Council on Clinical Cardiology and a member of the American Society of Echocardiography and is on the staff of NCH.

  • ARIEL DE LA ROSA, MD FACC, FSCAI


    Dr. De La Rosa is a graduate of Mount Sinai School of Medicine in New York, NY. He completed his Internal Medicine Residency, Cardiology Fellowship and Interventional Cardiology Fellowship at the Baylor College of Medicine in Houston, Texas, and then went on to practice as an Interventional Cardiologist in Miami, Florida where he was on the staff of University of Miami Hospital, Jackson Hospital System, South Miami Hospital, Mercy Hospital of Miami, and Palmetto General Hospital.  Dr. De La Rosa is Board Certified in Internal Medicine, Cardiovascular Diseases, and Interventional Cardiology. He is also a member of the American College of Cardiology, the American College of Physicians, and the Society for Cardiovascular Angiography and Interventions. Dr. De La Rosa relocated to Naples in 2014 and is currently on the staff of NCH and Physicians Regional Hospital.

  • SAJAN RAO, MD FACC

    Dr. Rao 
came to the group from Georgetown/ Washington Hospital Center in Washington, D.C. He is a graduate of George Washington University School of Medicine in Washington D.C. and completed his Internal Medicine Residency at the University of California - San Diego. Dr. Rao then completed a Preventive and Integrative Cardiology Fellowship at the Scripps Medical Center in La Jolla California. He went on to complete his Fellowship in Cardiovascular Disease at the Georgetown University/ Washington Hospital Center in Washington, D.C. After completing his training Dr. Rao was a member of the academic faculty at Georgetown University and Washington Hospital Center. In that role, he was active in caring for patients with complex cardiovascular disorders and advanced heart failure, along with running a large lipid clinic and being a member of a nationally recognized cardiovascular imaging team. Dr. Rao is Board Certified in Internal Medicine and Cardiovascular Disease and is a Diplomate with the National Board of Echocardiography and the Certification Board of Nuclear Cardiology. He is also a member of the American College of Cardiology, American Society of Echocardiography, and the National Lipid Association and is on the staff of NCH. Dr. Rao is currently on the staff of NCH and is a Partner in the practice. He was an NCH Physician of the Year Honoree and was named to Castle Connolly's Top Doctors in 2014 and 2015. His special interests include preventive cardiology, cardiovascular imaging, heart failure, and critical care.

  • HASHEM AZAD, DO

    Dr. Azad 
received his undergraduate degree from University of Miami and his medical degree from Nova Southeastern University. He completed his Internal Medicine Residency and Fellowship in Cardiovascular Disease at The Cleveland Clinic, where he also served as Chief Cardiology Fellow.  He is board certified in Internal Medicine, Cardiovascular Disease, Adult Echocardiography, and Nuclear Cardiology.  He is also board eligible in Vascular Ultrasound and Cardiac CT. He is a member of the American College of Cardiology, American Society of Echocardiography, and the American Society of Nuclear Cardiology.

  • JORDAN BEAVERSON, PA-C

    Jordan 
is a graduate the Florida Gulf Coast University College of Health Professions where he received his Bachelors of Health Sciences. He graduated suma cum laude from Nova Southeastern University with a Masters of Medical Sciences and as a Physician Assistant. While enrolled at Nova Southeastern University, Jordan conducted his cardiology rotation at SWICFT under the instruction of Dr. Rao and Dr. Talano. He joined the practice in 2015. Jordan s a member of the American College of Cardiology.
