  Dual Board Certified Internal and Geriatric Medicine Dr. Darstek is dual board-certified in Internal Medicine and Geriatrics by the American Board of Internal Medicine. She completed her Internal Medicine residency at Monmouth Medical Center in Long Branch New Jersey and her Geriatics fellowship at NYU Medical Center in New York City. Dr. Darstek has over 16 years of clinical experience during which time she has served as the Chief of Geriatrics at Monmouth Medical Center, has published multiple articles and papers, and lectured on many topics such as Dementia, Depression, Osteoporosis and Women's Health. Dr. Darstek believes that each patient should have a customized medical plan that fits their individual needs. She is dedicated to providing her patients with the highest quality medical care with a personal touch.

  James Joseph Faremouth, Jr.

  Board Certified Family Medicine and Osteopathic Manipulation Dr. Faremouth has a wealth of experience as a concierge medical doctor and private practice physician. Dr. Faremouth has over 18 years of experience, eight of which have been practicing in Naples, FL. He is originally from Grosse Pointe, Michigan, and completed his medical training at the Chicago College of Osteopathic Medicine in Chicago, Illinois followed by residency in Detroit, Michigan. Dr. Faremouth is board certified in Family Medicine and Osteopathic Manipulation. He also has specialized training in aesthetic procedures such as Botox. He treats the entire patient with a balanced approach and is trained to put the patient first not the specific illness. Dr. Faremouth is a great addition to our team. We are very excited to welcome him to Personalized Physician Care and look forward to you meeting with him.

  Natalia Keyser

  Dual Board Certified Internal Medicine/Hospice and Palliative Medicine Dr. Keyser is dual board-certified in Internal Medicine and Hospice and Palliative Medicine by the American Board of Internal Medicine. She completed her Internal Medicine residency at Temple University/St. Luke’s Hospital, Bethlehem, Pennsylvania and subsequently received additional training for her board certification in Hospice and Palliative Medicine. Dr. Keyser has over 12 years of clinical experience during which time she has served as the Medical Director of Vitas Innovative Hospice and Barrington Terrance Assisted Living Facility. She has published multiple articles and papers, and lectured on many topics such as Dementia, Diabetes, Cardiomyopathy, Thyroid Disease, and COPD. Dr. Keyser has over 10 years of concierge and private practice experience and prides herself of building close physician/patient relationships. Dr. Keyser believes that each patient should be treated as an individual and every patient needs a customized health care plan. She is dedicated to providing her patients with the highest quality medical care with a personal touch.
